Beijing couples who qualify to have a second child can download the birth permission application forms by logging onto the official website of the Beijing Municipal Commission of Population and Family Planning, the Beijing Times reported.
Required documentation includes the spouses' marriage certificate and birth certificates, at least one of which must show proof of being an only child. Applicants must submit the documents to government offices in their neighborhood. Fang Laiying, chief of the commission, warned that couples who are found to have submitted inauthentic documentation will be fined up to 800,000 yuan ($130,170).
According to the Beijing News, qualified couples who voluntarily relinquish their opportunity to have a second child can receive a one-time bonus of at least 500 yuan per person.
